DeWeese Health Center Financial Policy
Payment is expected at the time of service. The DeWeese Health Center accepts cash, check, VISA/Mastercard/Discover, and Flashcard. All unpaid charges will be remitted to the patient's 麻豆影院 State Student Account.
If you need financial assistance with your health center charges, please call Insurance and Billing at 330-672-8251.
A $20 NO SHOW fee will be assessed in ALL clinics if you miss a scheduled appointment.
INSURED PATIENTS
Insured patients are eligible KSU students, faculty, or staff who have health insurance coverage and elect to use it for services rendered at the DeWeese Health Center.
The DeWeese Health Center is a primary care PPO provider for insurance carriers that contract with the DeWeese Health Center. This list includes Aetna, Anthem BC/BS, CIGNA, Medical Mutual of Ohio, UnitedHealthcare and many others.
The DeWeese Health Center will submit claims to all insurance companies, regardless of our network participation.
1. OFFICE VISITS
Office Visits refer to evaluation and management services provided to patients in our medical clinics.
Copays, Co-Insurance and Deductibles
Insurance copays are due at the time of service and vary by insurance plan. Patients are also responsible for deductibles and co-insurance as indicated on the insurance explanation of payment.
2. LABORATORY SERVICES
The DeWeese Health Center provides onsite laboratory services for basic tests such as urine analysis, COVID-19, strep, mono, and HIV. Patients are responsible for deductibles and coinsurance as indicated on the insurance explanation of payment.
The DeWeese Health Center contracts with Quest Diagnostics as a reference lab for all tests that cannot be performed at the DeWeese Health Center. Quest will bill your insurance directly and you may receive a statement from Quest for the patient responsibility portion of your laboratory charges, separate from any charges you may receive from the DeWeese Health Center.
3. PROCEDURES, SERVICES, AND SUPPLIES
Procedures and Services include medical procedures performed in the medical clinics, as well as services rendered by the DeWeese Health Center's Radiology and the Immunization Clinic. Supplies include orthopedic devices and other durable medical equipment. Patients are responsible for deductibles, co-insurance, and non-covered services as indicated on the insurance explanation of payment.
4. PATIENT BALANCES AFTER INSURANCE
Students: The patient's Student Account will be charged if a patient balance remains after the insurance has paid.
Faculty/Staff: A billing statement will be mailed to the patient if a patient balance remains after the insurance has paid.
5. PHARMACY
The DeWeese Health Center Pharmacy participates in the Caremark Prescription plan network. Your Caremark copay applies and is payable at time of service at the checkout desk. Please provide your prescription insurance information to the pharmacy. You may have a separate prescription card, or the prescription insurance information may be included on your medical insurance card.
The DeWeese Health Center Pharmacy will fill all stocked prescriptions, but can only submit prescription claims to Caremark and the university's Student Health Insurance Program plan. The DeWeese Health Center Pharmacy offers competitive self-pay pricing for most prescriptions.
The DeWeese Health Center also carries many over-the-counter (OTC) supplies and items at below retail prices. OTC items are not billable to insurance and are payable at time of service at the checkout desk.
SELF-PAY PATIENTS
Self-Pay patients are patients who do not have health insurance or who elect not to use their health insurance.
1. OFFICE VISITS
Reduced fees for office visits are provided for self-pay patients in our medical clinics.
Charges for office visits are payable at time of service. Eligible students may also add charges to their 麻豆影院 State Student Account.
2. LABORATORY SERVICES
There are reduced fees for laboratory services provided by the DeWeese Health Center.
The DeWeese Health Center contracts with Quest Diagnostics as a reference lab for all tests that cannot be performed at the DeWeese Health Center. The DeWeese Health Center will bill self-pay students for the tests performed by Quest.
Charges for self-pay laboratory services are payable at time of service. Eligible students may also add charges to their Student Account.
If you have provided the DeWeese Health Center with your health insurance information, but you elect to utilize our self-pay rates rather than billing insurance for laboratory services, please notify both the lab and the checkout desk.

FINANCIAL CARE ASSISTANCE PROGRAM
The DeWeese Health Center will not turn away an eligible student because the student cannot pay for services. Pharmacy services and purchases are not eligible for the financial care assistance.
